Korean Treatments for Acne Scars

Acne scarring is one of the top reasons foreign patients travel to Seoul for dermatology. Korea has led scar treatment innovation for years, with access to devices and techniques that are either unavailable or new to Western markets.

But not all scars respond to the same treatments. Here's what actually works, based on scar type.

Understanding Your Scar Type

Ice pick scars: Deep, narrow, V-shaped pits. The hardest to treat. Best approached with TCA CROSS (chemical reconstruction of skin scars) followed by laser resurfacing.

Boxcar scars: Broad, rectangular depressions with defined edges. Respond well to fractional lasers (Fraxel, CO2 fractional) and microneedling RF (Morpheus8, Sylfirm X).

Rolling scars: Wide, shallow depressions with soft edges. Often the most responsive to treatment. Subcision (breaking fibrous bands under the skin) combined with fillers or Rejuran S produces excellent results.

Post-inflammatory hyperpigmentation (PIH): Dark marks from healed acne — not technically scars, but often mistaken for them. Respond well to pico lasers, chemical peels, and brightening skin boosters.

Korean Treatment Approaches

Fractional Laser Resurfacing

Creates microscopic columns of damage in scar tissue, triggering collagen remodeling. Korea's clinics use the latest fractional CO2 and erbium lasers with precision settings optimized for minimal downtime. Seoul price: ₩200,000–₩500,000 per session.

Microneedling RF (Sylfirm X, Morpheus8)

Combines microneedling with radiofrequency energy to stimulate deep collagen production. Particularly effective for boxcar and rolling scars. Korean clinics have extensive experience with these devices — some performing 20+ treatments daily. Seoul price: ₩250,000–₩600,000 per session.

Subcision + Rejuran S

A Korean favorite for rolling scars. The dermatologist uses a needle to break the fibrous bands tethering the scar down, then injects Rejuran S (the scar-specific PDRN formulation) to promote healing and prevent re-tethering. This combination is highly effective and largely unavailable outside of Korea. Seoul price: ₩300,000–₩500,000.

TCA CROSS

For ice pick scars specifically: trichloroacetic acid is applied precisely to the base of each scar, triggering controlled destruction and new collagen formation. Simple, effective, and performed expertly at Korean clinics. Seoul price: ₩100,000–₩200,000.

Realistic Expectations

Acne scar treatment is typically not a one-visit solution. Most protocols require 3–5 sessions spaced 4–6 weeks apart. A single Seoul visit can start the process — your first 1–2 sessions — with subsequent treatments either during return visits or with a dermatologist at home following the Korean protocol.

Expect 50–80% improvement with a full treatment course. Complete elimination of all scarring is rarely achievable, but the improvement is typically dramatic enough to significantly boost confidence and reduce the need for heavy concealer.
